“Fraud,” “abuse,” “upcoding,” “unbundling,” and “compliance” are common terms we all know but no one wants to talk about.  However, eliminating healthcare fraud and abuse has become a top priority for the federal government so we better get comfortable with it.  Poor clinical documentation and coding integrity practices can result in claim denials and inaccurate reimbursements. Many hospitals have implemented clinical documentation improvement programs to mitigate claim denials for medical necessity, coding, and clinical documentation.
